His photograph has been affixed to the document with two Violet Star of David and Jude stamped on the top centre of the front page. On the inside of the first page it mentions his parents and grandparents by name with the addition 'Jude' and 'Judin'.
This rare historical four sided document measures 8.5 x 12.25ins (22 x 31cm). It is in excellent condition and has been neatly folded in half at some time in the past

Besides the forms with the summons to collect the stamp peronally, there were similar forms with which the stamp was sent directly to recipients outside Prague. It was bilingual (German and Czech), with the following text: "Enclosed, we are sending you for the aforementioned person 1 admission stamp for one parcel to Terezin. Glue this stamp to the parcel. Parcels to Terezin without this stamp affixed, will not be forwarded by the mail. Please sign the receipt and return it immediatly. The Jewish Council of Elders in Prague Issuing office for admission stamps. 2 enclosures". The stamp was sent lightly affixed to the form. The forms with the stamp were sent, either in an envelope or without it, always registered, with a receipt. 5. A5/227a - G - 3n - III. 44 - 5/m - A. D. 348
In the space for applicant, the text "Von" and "od" are in one line: "Von -0 od". The printed address "Altestenrat der Juden in Prag. "And "Zidovska rada starsich v Praze" in lines below each other was placed below the text in the centre.

By the way, he is our respected secretary of our organization. The article is about' the one caught in Germany, the murderer of the vilna Jews, Martin Weiss'. May his name be erased! we are really asking very mach, because of the importance of the issue, for you to publish the article mentioned, into all the newspapers that you would have an opportunity to. Comments are needless". PS. [In script] "If because of certain local situations, you would be forced to change something in the article, you can do it".
